DOCUMENTATION NECESSARY TO REPORT ONBOARD
In
preparation for your Cadet Shipping experience, you need to secure the
following:
(Obtain
and submit all necessary documentation as indicated below if you have not
previously done so)

U.S.
Passport
You
will be required to enter the number of your U.S. Passport on your cadet
data sheet/billet preference form. (Choice
billets have been forfeited due to lack of passport).

Drug
Free Certificate
Drug Free
certificates are required and can be obtained from the medical department.

Medical
Records
Companies will require
documentation that you are fit for duty. Some companies request a copy of
your school entrance physical while others have their own medical review
requirements (DD Form 2807-1). Cadets
are also required to carry a shot card with them and if you are traveling
abroad additional shots may be required.

Benzene
Certificate
(Required
only for cadets shipping with companies that carry petroleum products)

U.S.
Merchant Marine Document (MMD)
Check with the
Registrar during the spring semester to ensure that they have received
all
the necessary information to facilitate processing of your MMD.

Basic
Safety Certificate
With
the implementation of STCW-95, many companies are requiring cadets
to have documentation for the four elements of Basic Safety Training:
Personal Survival (NS101 & PE114); Advanced & Basic Fire Fighting;
Basic First Aid; Personal Safety & Social Responsibilities (PD102 &
PD202). All students are required to have this certificate before leaving
campus.

Anti-Terrorism Certificate
September 11, 2001 has heightened terrorism awareness and implemented
procedures that are important when on vessel. The certificate that you
earned needs to be part of the documentation that is necessary when
reporting on board.
Make
every effort to improve your academic standing and disciplinary performance
starting from Day One. The results will have an important influence on where you are
placed for cadet shipping.
